This is a Contract position in Norfolk, VA posted November 21, 2021.

Job Summary: Completes monthly reviews of each Community Living individual and serves as a health/medical resource.ESSENTIAL FUNCTIONS OF THE JOB:Monitor health of each individual in Community Living program.

Review and process Medication Administration Records (MAR) monthly and controlled drug counts and physician orders to ensure accuracy.Communicate with CMRO RN and Program Managers to complete Nursing Review Action Plans.Serve as a resource for Program Managers who are coordinating care for individuals.Maintain ongoing professional communication with internal and external stakeholders.Medical delegations and training (insulin, smoking cessation, lifting, ostomy care).Develop medical protocols (choking, seizure, diabetes).Maintain prompt, accurate logs for hours worked for billing purposes according to Medicaid guidelines and identify if services are billable or not billable according to the Medicaid PAM guidelines.Complete AIMS testing as necessary.Write a monthly review based on physical assessment of individual and records for previous month.Attend CMRO nursing reviews.Act as a liaison during hospital discharge if needed.ACT is an equal opportunity employer and we believe in strength through diversity.
