– POSITION SUMMARY: The Respiratory Therapist provides respiratory services to Affinity Care Hospice patients under orders from the referring physician, so that the quality of the patients’ life may be enhanced.

The Respiratory Therapist functions in accordance with their state’s practice act, as applicable.

QUALIFICATIONS: 1.

Be a graduate of an approved education program to train Respiratory Therapists with a minimum of an Associates Degree.

2.

Minimum two years of experience as a Respiratory Therapist.

3.

NBRC (National Board for Respiratory Care) credentialed (CRT minimum; RRT preferred) 4.

RRT: Acute care and home care field experience required 5.

CRT: Minimum two years experience with at least one year acute care and one year home care field experience required.

6.

Current State License for Respiratory Care Practitioner in the state of practice, if applicable.

7.

Excellent oral and written communication and interpersonal skills; 8.

Demonstrated competency in applying the principles, methods and equipment used in respiratory therapy.

9.

Ability to function to access patient needs and develop and implement a comprehensive plan of care.

10.

Ability to function as a member of an integrated healthcare work team.

11.

Be a licensed driver.

RESPONSIBILITIES AND DUTIES: 1.

Reviews available information and evaluates impact on treatments.

2.

Performs a thorough assessment, evaluation and documentation of patient’s level of respiratory functioning in response to therapy.

3.

Develops and implements an individualized treatment plan in consultation with the physician and interdisciplinary team to promote optimal respiratory function related to the patient’s potential.

4.

Provides respiratory services in accordance with plan of care in a safe, timely manner.

5.

Instructs and consults with patients and caregiver in appropriate respiratory care procedures and techniques appropriate to their age and needs.

6.

Consults with physicians, other health care workers and caregivers as needed regarding patient progress, problems and plans.

7.

Recommends appropriate interagency and intra-agency referrals.

8.

Provides educational programs for Affinity Care Hospice staff members.

9.

Assists in the referral process when a patient requires additional rehabilitation after discharge and completes in full the discharge plan on each patient.

10.

Participates in peer review and quality improvement process, as directed.

11.

Participates in continuing education appropriate to discipline.

12.

Completes all annual mandatory training and certification requirements.

13.

Maintains a professional attitude and ethical approach to patients and organization.

14.

Supervises support staff and delegates duties where appropriate.

15.

Recommend appropriate interagency duties where appropriate.

16.

Goal setting based on needs of hospice patient/family.

17.

Communication of information to appropriate IDT/IDG members.

18.

Completes appropriate documentation in a timely manner.

19.

Participates in Quality Assessment and Performance Improvement Activities.

20.

Performs other duties as assigned and required to provide for quality care to patients.
